Keep the Pantheon, Trevi Fountain and surrounding streets together as a walking day. Put the Colosseum and Forum on another part of the programme, with the new Metro C interchange offering another way into that district. Trastevere fits more naturally with an evening across the Tiber than with repeated crossings between archaeological sites.
The Vatican Museums belong on their own half-day across the river. A central Rome hotel does not make every major attraction a neighbouring stop.
